Hospital Visitations

If you would like to have Fr. Peter visit your loved one in the Hospital for Holy Communion or Prayers, kindly fill out the request form below, stating the name of your relative who is hospitalized;  the Hospital and Room Number they are in; and your Name, email and cell number. If the matter is urgent, please do not wait until the last minute to contact the Priest, as Holy Communion can only be administered if the patient is able to swallow.
