Sick & Shut-In Visitations

Our Lord commands us to never forget those who are sick and housebound and to minister to them:  "I was sick and you visited me!" (Matthew 25:36)  If you have an elderly or sick loved one at home that would like to receive Holy Confession and Communion or any other blessing, kindly fill in the request form with the name of the loved one, their Address and your name, email and cell number so that an appointment may be set up.
